Abstract: Background Uterine torsion is a very rare gynecological emergency and is defined as torsion of the uterus greater than 45° around its axis. Case presentation This case report presents a 68-year-old postmenopausal woman with complaints of generalized abdominal pain and distension for 4–5 months. Ultrasonography and MRI (Pelvis) was suggested a Right ovarian mass of size 24 × 14x19cm. The patient underwent exploratory laparotomy with Total abdominal hysterectomy and bilateral salpingo-oophorectomy. Intraoperatively, there was evidence of Right ovarian mass of size 24 × 14x19cm with bulky necrotic uterus of 14 weeks size with 360 degree torsion over its axis. Total abdominal hysterectomy was performed without detorting the uterus. The procedure went uneventful without complications. Conclusion Uterine torsion is a diagnostic dilemma as it has no specific sign or symptom and is asymptomatic in many cases. The management of this disease differs depending upon the age, fertility, parity and extent of ischemia, but laparotomy is the gold standard in all cases.
