Abstract: INTRODUCTION: This retrospective, single-center study of consecutive eyes evaluated early visual and refractive outcomes following keratorefractive lenticule extraction (KLEx) using VISUMAX 800 femtosecond laser (Carl Zeiss Meditec) for myopia and myopic astigmatism in a large population. METHODS: Adult patients with a myopia up to - 10.00 D and astigmatism up to 5.00 D were treated in this study. Uncorrected distance visual acuity, manifest refraction, corrected distance visual acuity (CDVA), and high-order aberrations (HOA) were assessed at the 3-month postoperative visit. RESULTS: In total, 1611 eyes were included in this study. Mean preoperative manifest spherical equivalent (SEQ) was - 4.49 ± 1.87 D. At the 3-month postoperative visit, UDVA was 20/20 or better in 99% of eyes, compared to 100% preoperatively. The achieved SEQ was within ± 0.50 D of the intended correction in 98.7% of eyes, and within ± 1.00 D in 99.8%. Residual refractive astigmatism was ≤ 0.50 D in 97.3% of eyes. Mean postoperative total HOAs was 0.13 ± 0.12 µm. No eyes lost two or more lines of CDVA. CONCLUSION: Early outcomes following KLEx using the VISUMAX 800 demonstrated excellent accuracy and predictability in the treatment of myopia and myopic astigmatism.
