A study focusing on percent agreement of evaluations obtained by videoendoscopic examination of swallowing with those by videofluoroscopic examination of swallowing conducted on the same day

Type: dissertation
Study design: Observational Study
Abstract: Background: Videoendoscopic examination of swallowing (VE) and videofluoroscopic examination of swallowing (VF) are considered "gold standard" diagnostic assessments to evaluate swallowing disorders (dysphagia). Moreover, a correlation between VE and VF has been reported in many scientific articles. However, the intrajudge reliability between VE and VF is still unclear. The aim of this study was to evaluate the intrajudge reliability between VE and VF in clinicians with/without experience in dysphagia management.Material & Methods: 34 patients who had undergone VE and VF on the same day from October 2019 to April 2020 at the Teikyo University Hospital were included. The VE and VF data were edited focusing on each target pool, swallowing function, and reflex. The criteria of the VE and VF data were judged after discussion by three dentists who had an authorization from the Japanese Society of Dysphagia Rehabilitation (JSDR) and had an experience in the management of dysphagia more than ten years (richly experienced dentist: RED).17 raters were divided into two groups: one group with seven raters who had an authorization from the JSDR (authoraized dentist: AD), and the other with 10 raters who did not have an authorization (novice dentist: ND).The intrajudged reliability between VE and VF were compared with AD in ND.Result: The intrajudge reliability between VE and VF was 68.91% in AD and 62.64% in ND.The inter-rater agreement rate between RED and AD was 76.89% for VE and 78.15% for VF.The inter-rater agreement rate between RED and ND was 67.64% for VE and 77.05% for VF.Conclusion: The results of this study indicate that VE and VF findings assessed under the same test conditions were different regardless of the clinician's experience. Both the VE and VF should be undertaken to complement difference of findings between both examinations.
