Abstract: Recurrent gliomas, whether isocitrate dehydrogenase (IDH) mutant or wildtype, have limited effective treatments, supporting evaluation of biomarker-guided therapies. IDH mutations drive accumulation of 2-hydroxyglutarate, creating vulnerability within DNA damage response pathways by inducing homologous recombination deficiency (HRD). In preclinical models, HRD has been associated with sensitivity to poly (ADP-ribose) polymerase (PARP) inhibition and appears reversible with IDH inhibition. Accurate characterisation of HRD in gliomas may inform its utility as a therapeutic biomarker.
