Rare and major conduction system pacing complications as an alert for our general awareness

Abstract: The electrophysiological and anatomical rationale for conduction system pacing (CSP), first described still in the 60-70s is currently experiencing a renaissance.CSP is rapidly gaining position and evolving into the standard of care for permanent cardiac pacing, replacing other right ventricular lead locations or competing for outcomes with cardiac resynchronization in selected populations [1,2].Despite a surge in publications in this field, the literature remains dominated by observational studies [3], specific population results [4] and case series. From an evidence-based perspective, there is a paucity of large-scale randomized controlled trials with long enough follow-up in specific target populations that evaluate all aspects of permanent pacing, including procedural complications and the maintenance of expected capturespecificity and electromechanical synchrony.A retrospective observational study by Rojna et al. [5], involving a large multicenter cohort, highlights one of the acute traumatic aspects that may be associated with the left bundle
