Abstract: The synergy between DNA topoisomerase-1 inhibitors (TOP1i) and DNA-damage response inhibitors (DDRi) has potential to improve efficacy, but overlapping toxicities limit clinical utility. The selective delivery of TOP1i to cancer cells by antibody-drug conjugates (ADC) has renewed interest in these combinations. Therefore, we aimed to identify which specific DDR pathway inactivation best sensitizes to TOP1i-ADCs, assess predictive biomarkers of efficacy, and determine if TOP1i-ADCs can address limitations of systemic chemotherapy-DDRi combinations.
