Abstract: • A sit-to-stand (STS) assessment is a simple test and surrogate marker of frailty. • Below average STS is associated with more complications, re-operations, readmissions. • STS may to help risk-stratify patients preoperatively. We aimed to investigate the utility of the validated frailty sit-to-stand (STS) measure preoperatively as a predictor of postoperative outcomes after robotic hysterectomy (RH). This was a prospective, observational study of adult women who underwent a RH within our institution from 01/2022 to 06/2023 by a gynecologic oncologist. A 30 s STS was performed preoperatively, and patients were categorized as above or below average performers by age. Comparative statistics were employed. A multivariable Firth’s logistic regression model evaluated a composite primary outcome of complication, re-operation, or readmission. A total of 175 patients participated in this study, including 38 below average STS performers (12%) and 137 above average performers (88%). There were no differences in demographics, medical comorbidities, surgical history, or procedure-related times between the groups. Complications were significantly higher in the below versus above average STS group (13% versus 0.7%, p < 0.001), as was the rate of re-operation (8% versus 0%, p < 0.001) and readmission (16% vs 2%, p = 0.004). The primary composite outcome was statistically significant with more events occurring in the below average STS group (16% versus 2%, p = 0.004). This remained significant when adjusting for age, race, presence of any comorbid disorder, and any prior abdominal surgery with an OR of 9.50 (95% CI: 2.29–56.5; p = 0.002). A below average STS assessment was predictive of a worse composite outcome of complication, re-operation, and readmission. Preoperative STS assessment may be a quick, effective tool to help risk-stratify, inform treatment planning and aid in risk discussion.
