A Multi Method Comparative Psychometric Assessment of Two Pain Catastrophizing Measures: The Concerns About Pain and Pain Catastrophizing Scales
Year: 2026
Type: preprint
Abstract: This project will conduct a comprehensive psychometric comparison of the Concerns About Pain (CAP) scale and the Pain Catastrophizing Scale (PCS) to clarify their statistical relationship and evaluate their performance in a clinical setting. Because both instruments are developed to assess the construct of pain catastrophizing, the project will examine the extent to which they capture overlapping or distinct dimensions of the construct, as well as whether they demonstrate similar or differential clinical performance. The current study will also systematically compare the two measures with respect to their validity, reliability, and clinical performance, including sensitivity to change and factors influencing change (e.g., mediational mechanisms underlying pain treatment effects). Finally, we aim to explore whether the assessment of pain catastrophizing can be further refined by identifying items from the CAP, the PCS, or a combined item pool that optimizes psychometric properties and clinical performance among patients receiving pain treatment in a tertiary pain clinic.
