Abstract: Background: Interstitial (cornual) pregnancy is a rare type of ectopic gestation associated with delayed rupture and life-threatening hemorrhage. Case Presentation: A 34-year-old G3P2L2 woman presented at 16+3 weeks with prolonged vaginal bleeding and hemodynamic instability. Ultrasound revealed an empty uterus and a 15–16-week fetus in the pouch of Douglas with massive hemoperitoneum. Emergency laparotomy confirmed ruptured left cornual pregnancy. Conservative surgery failed, necessitating subtotal hysterectomy. Conclusion: Cornual pregnancy is diagnostically challenging and may present late with catastrophic bleeding. Prompt surgical intervention is essential in unstable patients.
