Abstract: . Objectives: To characterize temporal trends in amyloid kidney disease among people who inject drugs (PWID) and evaluate associated renal and mortality outcomes. Methods: We conducted a retrospective cohort study using TriNetX data from 2018 to 2025. Trends in amyloid kidney disease with proteinuria or chronic kidney disease (CKD) stage ≥3b were assessed among PWID. Outcomes were compared between PWID with and without renal involvement using propensity score-matched analyses. Results: Age-adjusted incidence of amyloid kidney disease increased from 146 to 238 per 100,000 person-years between 2018 and 2025 (annual percent change 6.8%), while prevalence nearly doubled. Among affected individuals, dialysis incidence increased from 2.04 to 8.83 per 100 person-years. After propensity score matching, renal involvement was associated with higher hazards of death (HR 1.80, 95% CI 1.44,2.25) and dialysis or death (HR 2.54, 95% CI 2.02,3.19). Risks of skin and soft tissue infection and sepsis were similar between groups, while venous thromboembolism or catheter-associated bloodstream infection was modestly increased. Conclusions: Amyloid kidney disease among PWID is increasingly recognized and associated with substantially higher risks of dialysis and death. Earlier recognition of renal involvement may provide opportunities to prevent or slow progression.
