Abstract: Kartagener syndrome is defined as a primary ciliary dyskinesia with visceral inversion, bronchiectasis, and chronic sinusitis. Male patients with primary ciliary dyskinesia including Kartagener syndrome are also associated with male infertility, mainly because of sperm tail immobility. We report a case in a 36-year-old man with Kartagener syndrome and azoospermia. An examination indicated obstructive azoospermia. Therefore, testicular sperm extraction and microsurgical epididymal sperm aspiration were performed, and sperm retrieval was successful. Motile epididymal spermatozoa were used for the first intracytoplasmic sperm injection, and a single embryo was transferred. The course of pregnancy was uneventful and a healthy boy was delivered. Intracytoplasmic sperm injection following microsurgical epididymal sperm aspiration is an effective treatment for infertility due to Kartagener syndrome.
