Abstract: BACKGROUND: Chronic obstructive pulmonary disease (COPD) ranks as the fourth leading cause of death globally, and studies indicate that physical activity can benefit patients. However, many struggle to maintain activity levels due to symptoms like dyspnea. The "Healthy China 2030″ initiative encourages physical activity, yet there remains a need to understand the current state of participation among COPD patients and explore health education as a behavioral promotion factor. OBJECTIVES: To investigate participation behavior in physical activity and its influencing factors in individuals with COPD. METHODS: A cross-sectional design was employed. Four nursing graduate students conducted surveys using a self-developed general information form and the COPD Physical Activity Participation Behavior Assessment Scale. Univariate analysis and multiple linear regression were used to analyze factors influencing physical activity participation among COPD patients. RESULTS: A total of 274 valid questionnaires were collected, reflecting a 90.43% response rate. Among respondents, 54.75% were aged 70 or older, 83.21% were male, and 67.52% lived in urban areas. The mean score on the COPD Physical Activity Participation Behavior Scale was 126.55 ± 15.48. Multiple linear regression identified several key factors influencing participation: place of residence, disease stage, duration of diagnosis, willingness to engage in physical activity training, role of healthcare professionals in providing physical activity education, and frequency of receiving such education. CONCLUSION: COPD patients in China demonstrate moderate levels of physical activity participation. Despite a strong willingness and some opportunities, their actual engagement in physical activities is limited, underscoring the need for enhanced participation behavior strategies.
