Abstract: Variceal bleeding can be a fatal complication of decompensated alcoholic cirrhosis, especially in patients presenting with an acute operative pathology. Cystic artery bleed leading to a ruptured gallbladder is a rare but possibly fatal presentation in patients with portal hypertension secondary to alcoholic liver cirrhosis. Here, we present a case of cystic artery hemorrhage leading to a hemorrhagic cholecystitis, ruptured gallbladder, hemorrhagic shock, and hemoperitoneum, requiring in an emergency exploratory laparotomy and open cholecystectomy.
