Abstract: Chronic insomnia is a complex reason for seeking primary care and exposes patients to prolonged use of hypnotic treatments despite their known risks. Cognitive behavioral therapy for insomnia (CBT-I) is the recommended first-line treatment, but access to it remains limited. This systematic review evaluated the effectiveness of group CBT-I in reducing insomnia, using the ISI (Insomnia Severity Index) score in patients with chronic insomnia in primary care. Twenty-one studies were included. Eleven studies showed a reduction in the severity of insomnia with a significant decrease in the ISI score (loss of 2.5 to 8 points). Some patients even achieved remission, with an ISI score <8. The results were maintained during follow-ups up to two to four years after the intervention. The other ten articles evaluated the reduction in insomnia using other subjective and objective scores. These improvements were accompanied by positive effects on mental and physical quality of life. In summary, group CBT-I appears to be effective in reducing the severity of insomnia in primary care settings among patients suffering from chronic insomnia.
