Abstract: Background: Open ventral hernia repair in obese patients carries considerable morbidity. Laparoscopic IPOM (Intraperitoneal only mesh) repair is associated with severe postoperative pain and adhesion obstruction than ventral TAPP (transabdominal pre-peritoneal) repair. But ventral TAPP has a long learning curve. The aim of the present study was to compare the feasibility and efficacy of ventral TAPP with IPOM plus in the treatment of ventral hernia in obese patients. Methods: This prospective comparative study was carried out in surgical department of Bahawal Victoria Hospital, Bahawalpur from 15-10-22 to 31-12-24. 44 Patients were equally divided into groups A and B by simple random sampling. In Group A, laparoscopic ventral TAPP and in group B laparoscopic IPOM plus was performed. Data was analysed using SPSS Statistics version 27. The Pearson chi-squared test was used for the categorical variables and independent T test for continuous variables. A p-value <0.05 was considered statistically significant. Results: Mean operating time was significantly longer in group A than in group B ((96.27± 6.288 min vs. 79.09± 6.288 min, p value < 0.001). Postoperative pain (VAS score) at days 1, 2, and 15 was more severe in group B than in group A. The complication rate was higher in group B than in group A. (13.65% vs. 4.55%). Mean hospital stay was shorter in group A than in group B (2.545 ± 1.01 vs. 3.73 ± 1.08 days). Conclusion: Although ventral TAPP in obese patients is technically demanding and has a long learning curve but it has lower postoperative pain, complications rate and hospital stay. Ventral TAPP may be a better alternative to IPOM plus.
