Abstract: Background: The most common adult upper limb fractures are the proximal humerus fractures (PHFs). Systems of plates, such as the Proximal Humerus Internal Locking System (PHILOS) have gained popularity in providing angular stability and early mobilization. The objective of the study was to determine the functional and radiological results of PHFs that were surgically treated with open reduction and internal fixation (ORIF) with PHILOS plating in a local adult population. Methods: The present descriptive longitudinal study included 140 adult individuals with closed PHFs treated with ORIF using PHILOS plates. The Constant-Murley score was used to assess the functional outcome at 3 months postoperatively. Radiological union, demographic variables, comorbidities, operative duration, and hospital stay were analysed. Factors leading to poor functional outcomes were analysed by applying Logistic regression. Results: Most patients were male (62.1%) and the average age of the patients was 55.0 ± 13.7 years. Functional outcomes were graded as excellent (31.4%), good (27.1%), moderate (25.0%) and poor (16.4%) of patients. There were 90.7% of patients who achieved radiological union at the end of 12 weeks postoperatively. On univariate analysis, BMI ≥25 and prolonged surgery (≥60 minutes) were associated with poor outcomes (p<0.25). The multivariate analysis showed that individuals with BMI <25Kg/m2 are 76% less likely to have poor outcome as compared individuals with BMI ≥25 kg/m² (AOR: 0.24; 95% CI: 0.085–0.657). whereas operative duration ≥60 minutes was associated with higher odds of poor outcome (AOR: 3.303; 95% CI: 1.198–9.105). Conclusion: ORIF using PHILOS plates yields favourable outcomes in PHFs. BMI and Duration of surgery needs to be monitor closely as it leads to poor outcome.
