Abstract: Pulmonary hydatidosis is a parasitic infection caused by Echino co ccus granulosus, m o s t co m m o n l y a f f e c t i n g immunocompromised individuals.Lophomonas blattarum is a rare emerging pathogen that affects the lungs.Coinfection with these two organisms is a rare entity, presenting as a lower respiratory tract infection with no specific diagnostic tests available. In this report, a combined approach involving clinical history, diagnostic evaluation, and management is discussed. case descrIptIonA 69-year-old male patient was admitted in the Department of Pulmonology at our institute with complaints of grade 2 dyspnea, on and off fever, loss of appetite, vomiting, and history of wheezing since 1 month. The patient had recurrent respiratory infection since the past 6 months, for which he was managed medically on an outpatient basis. He also had a history of Echinococcus infection in the liver 15 years ago, for which medical management was done.On admission, the patient's vitals were stable with PR = 86 bpm, BP: 130/80 mm Hg, RR: 24 cpm, and SpO 2 : 94% at RA. Systemic examination showed bilateral expiratory wheeze on auscultation.Routine lab tests showed eosinophilia (22%), with absolute eosinophilic count 2750, total leukocyte count = 12500.The ESR (erythrocyte sedimentation rate) was 10 mm.Sputum smear was negative for tuberculosis and fungi, and sputum culture revealed no growth.Chest X-ray s/o left lower lobe consolidation/mass (Fig. 1), hence he was treated with beta lactam antibiotics. No response to the antibiotic therapy was seen, hence for further evaluation HRCT was advised, which revealed a large multiloculated cystic lesion of size 6.2 × 9.6 × 8.8 cm in the left lower lobe with multiple cysts in the spleen, likely pulmonary hydatid cyst (Figs 2 to 4).The patient was investigated further with bronchoscopy, which showed narrowed opening of left lateral lower lobe segments and congestion with mucoid secretions infective/malignancy.
