Repurposing a thoracic branch endoprosthesis side branch for complex aortic pathology

Abstract: We report a novel use of the Gore thoracic branch endoprosthesis (TBE) side branch component for treating complex aortic pathologies. Two patients with high-grade midthoracic aortic stenosis underwent repair using the TBE side branch as a tapered stent graft combined with distal balloon-expandable stenting to coarctation anatomy. A third patient with a large anastomotic aneurysm of a visceral bypass underwent physician-modified endograft repair incorporating the TBE side branch as an antegrade branch into a dilated Dacron surgical graft. All repairs were technically successful, with symptom resolution and durable short-term outcomes.
