Abstract: Background: Chronic non-specific low back pain is a common condition and a leading cause of pain and functional disability among people of working age. The condition is understood from a biopsychosocial perspective, in which both physical and psychological factors contribute to the persistence of pain. Consequently, international clinical guidelines recommend combined treatment approaches involving physical exercise and cognitive behavioral therapy (CBT). However, evidence regarding the effects of such combined interventions within physiotherapy practice remains limited. Purpose: To investigate whether physical exercise combined with CBT principles is more effective than physical exercise alone in adults with chronic non-specific low back pain Methods: A systematic literature review was conducted using the databases PubMed and PEDro. Quality was assessed using the SBU risk-of-bias tool RoB 2. The certainty of evidence was assessed using the SBU GRADE system. Results: Six RCT-studies were included. Improvements in pain intensity, functional disability, and fear of movement were observed in both intervention and control groups. Significant differences in favor of combined treatment were reported in three studies for pain intensity and fear of movement, and in two studies for functional disability, while two studies found no significant differences. Overall, the level of evidence was assessed as moderate. Conclusions: The findings suggest that physical exercise combined with CBT-based components delivered within physiotherapy may be beneficial compared to physical exercise alone; however, the evidence is limited and further research is needed.
