Abstract: Pentalogy of Cantrell (PoC) is a congenital condition characterized by midline thoracoabdominal defects. We report a neonate presenting with mesocardia and a left ventricular diverticulum repaired in a 2-staged approach. Imaging confirmed cardiac and thoracoabdominal defects typical for pentalogy of Cantrell. The histopathology of the diverticulum demonstrated myocardial components with endothelial lining. This case highlights the diagnostic value of multimodal imaging, potential benefits of a staged repair approach, and provides insights into cardiac diverticula associated with pentalogy of Cantrell.
