Management of Complicated Crown–Root Fracture

Study design: Case Report
Abstract: Crown–root fractures are traumatic injuries that frequently extend below the gingival margin and present restoration complications due to biologic width violations, isolation issues, and structural loss. In this case report, a 16-year-old boy presented with fractured maxillary lateral and central incisors 24 h after trauma. Cone-beam computed tomography imaging revealed a complex crown–root fracture of the tooth with palatal subgingival extension. After stabilizing the fragment, root canal treatment was performed, a full-thickness flap was elevated, and the margin was exposed using osseous recontouring. A crown made of lithium disilicate was luted with resin cement. A 1-year follow-up revealed stable periodontal and periapical health with satisfactory esthetics and function.
