Conductive Scaffold for Spinal Cord Regeneration Bridge (CSRB): A Four-Component Fiber System with Dual Inhibition Release, Topographic Axon Guidance, and Remyelination for Complete and Incomplete Spinal Cord Injury
Year: 2026
Type: article
Abstract: No FDA-approved therapy restores function after chronic complete spinal cord injury (SCI) as of 2026. Five simultaneous barriers prevent CNS axon regeneration: (1) CSPG glial scar, (2) Nogo-A myelin inhibition, (3) absence of aligned physical substrate, (4) axon misrouting, (5) demyelination. CSRB addresses all five in a single implant. Component 1: PEDOT:PSS electrospun nanofibers (200-500 nm, rotation-aligned ≥90% within 5 degrees, fibronectin/laminin coated) for contact guidance and electrical stimulation. Component 2: Dual sustained-release system — ChABC (chondroitinase ABC, Bradbury et al. 2002 Nature) dissolves CSPG barrier; NEP1-40 (Nogo receptor antagonist, GrandPre et al. 2002 Nature) blocks myelin inhibition. Together approximate PNS regeneration environment in CNS. Component 3: Topographically segregated neurotrophin gradients — BDNF for corticospinal tract zone (dorsal/lateral), NT-3 for spinothalamic tract zone (anterior), spatial separation ≥2 mm, PLGA microsphere 30-day release, prevents motor/sensory axon misrouting. Component 4: Two-layer cell seeding — iPS-derived neural stem cells (1×10⁶/10mm, dual SMAD inhibition protocol) + iPS-derived OPCs (5×10⁵/10mm, Wnt/BMP+PDGF-AA protocol) for remyelination (Kang et al. 2013 Cell Stem Cell confirmed iPS-OPC remyelination in SCI). Two-phase protocol: acute (Day 0-Week 4, scaffold insertion, ChABC/NEP1-40 release) + subacute (Week 4-24, cell seeding, epidural EES 40 Hz, neurotrophin maintenance). Quantitative endpoints in T10 complete transection rat: BBB score ≥10/21 at 12 weeks, ≥15/21 at 24 weeks; MEP re-emergence ≤150% normal latency; axon density ≥40% normal; remyelination ≥60%. V=N/D framework: maximize N (fiber alignment × neurotrophin gradient × cell density) while D→0 (ChABC dissolves CSPG, NEP1-40 blocks Nogo-A, minimal surgery). Theoretical design framework only. All animal and human trials by manufacturing partners. FDA pathway: BLA + PMA combination product. Estimated approval: 2038-2040. Fourteenth paper in unified series: ECCE/CPMH/SIBM/GMFT/FICI/CFOG/CGFT/FJGT/Review/CFLR/KokuUniverse/LivingArchitecture/CosmologyReview/CSRB. TheYKHC 2026.
