Real-world treatment patterns and outcomes with subsequent treatments following frontline brigatinib: a retrospective chart review

Year: 2026
Type: article
Abstract: This chart-review study retrospectively evaluated real-world treatment patterns and outcomes with subsequent treatments following frontline brigatinib in ALTA-1L. Patients previously enrolled in ALTA-1L were followed after brigatinib discontinuation. Among 40 patients who received subsequent systemic anticancer therapies, 30 (75%) received 2L ALK tyrosine kinase inhibitors (TKIs; lorlatinib, n = 16; alectinib, n = 8; crizotinib, n = 6), and 11 (28%) received 2L non–ALK TKI therapy, one of whom received it with alectinib. Real-world overall response rate on 2L treatment was 33% and 0% with ALK TKIs and non–ALK TKI therapy, respectively. Median (95% confidence interval [CI]) real-world time to treatment discontinuation was 34.7 months (4.6–not reached [NR]) for 2L ALK TKIs (lorlatinib, 37.2 months [6.0–NR]; alectinib, NR [1.1–NR]; crizotinib, 2.8 months [2.0–NR]) and 4.4 months (1.4–6.0) for 2L non–ALK TKI therapy. Median (95% CI) real-world progression-free survival was 16.1 months (4.4–NR) with 2L ALK TKIs (lorlatinib, 25.6 months [3.8–NR]; alectinib, 16.1 months [1.1–NR]; crizotinib, 2.4 months [2.0–NR]) and 6.1 months (1.7–NR) with 2L non–ALK TKI therapy. In conclusion, most patients advanced to a second ALK TKI following frontline brigatinib, with second- or third-generation ALK TKIs providing prolonged clinical benefit after brigatinib.
