Q Fever Clinical Screening Framework: Evidence-Based Risk Assessment for Rural Populations

Year: 2026
Type: article
Abstract: This poster presents the Q Fever Clinical Suspicion Index (QFCSI), an evidence-informed screening framework designed to improve identification of Q fever among rural populations. Current screening approaches emphasize occupational exposure despite evidence that many reported cases occur among individuals without direct livestock contact or employment in traditionally high-risk occupations. Drawing upon findings from a systematic review of 81 global outbreaks, United States surveillance data, international seroprevalence studies, and case-control research, the framework integrates demographic, exposure, clinical, and medical-risk factors into a practical point-based assessment tool. The QFCSI is intended to support clinical decision-making, improve diagnostic suspicion, and encourage earlier identification of potential Q fever infections in community and rural healthcare settings. This framework is conceptual and requires prospective validation before clinical implementation.
