Reaching consensus in relapsed/refractory ALL in AYAs
Year: 2026
Type: editorial
Abstract: In this issue of Blood Advances, O'Dwyer et al 1 present consensus guidelines for the management of relapsed/refractory acute lymphoblastic leukemia (ALL) in adolescents and young adults (AYAs).Although outcomes for children with ALL have improved dramatically, survival for AYAs has lagged considerably, and this gap is especially pronounced in the setting of relapsed/refractory disease. To address this issue, the American Society of Hematology formed a multidisciplinary panel of key stakeholders in the care of AYAs, including patient representatives, to develop evidence-based guidelines using the Grading of Recommendations, Assessment, Development, and Evaluation approach to formulate the strength of recommendations. The panel reached consensus on 8 treatment recommendations focused on reinduction and consolidation therapy. The authors are commended on their comprehensive review and efforts to harmonize broad perspectives from providers in both pediatric and adult hematology/oncology.
