2026 AAPM-COMP MO-115-109 - Joint ESTRO-AAPM Session on Clinical Trial Design for Medical Physicists: Moving from QA into Oncologic Outcomes and Biomarker Applications
Year: 2026
Type: article
Abstract: Delivered 2026-07-19T1315 at the 2026 American Association of Physicists in Medicine (AAPM)/Canadian Organization of Medical Physics (COMP) Joint Meeting in Vancouver, BC, CA.Precis: This presentation equips medical physicists to participate in clinical and biomarker trial design beyond traditional quality assurance. It shows how endpoint selection, timing, clinically meaningful thresholds, and protocol compliance can determine conclusions, even when trials study similar technologies. Direct clinical endpoints, surrogate endpoints, and biomarkers must be distinguished; biomarkers characterize biology but cannot alone establish patient benefit. The talk examines time-to-event, composite, toxicity, patient-reported, quality-adjusted, and Bayesian endpoints, emphasizing their assumptions and potential distortions. Because radiotherapy effects are often modest, physicists should help prospectively integrate dosimetry, RBE, NTCP, imaging, confounder analysis, and adaptive methods into rigorous, interpretable, clinically relevant trials.
