Primary pulmonary osteosarcoma: a case report, literature review and pooled analysis

Year: 2026
Type: article
Abstract: Background: Primary pulmonary osteosarcoma (PPOS) is an exceptionally rare malignant tumor that arises within the lung parenchyma. Fewer than 40 cases of PPOS have been reported to date. Owing to its rarity, the clinicopathological characteristics and prognostic factors of PPOS remain poorly defined. Objective: To report a new case of PPOS and perform a pooled retrospective analysis of published cases to identify clinical profiles and potential prognostic determinants. Methods: A comprehensive literature review was conducted using the PubMed, Medline, and Web of Science databases. Thirty-seven reported cases were identified, of which 28, including one patient from our institution, provided sufficient clinical and survival data for analysis. Overall survival (OS) was assessed using the Kaplan–Meier method and Cox proportional hazards models. Results: The median age of the pooled cohort was 65 years. Multivariate Cox analysis demonstrated that older age (≥ 60 years; hazard ratio (HR) = 17.01; \(\:P\) = 0.002) and larger tumor size (≥ 7 cm; HR = 4.56; P = 0.045) were independently associated with poorer OS. Surgical resection significantly improved survival (HR = 0.16; P = 0.011). Among patients who underwent surgery, adjuvant chemotherapy was associated with a trend toward prolonged OS, although statistical significance was not reached ( P = 0.065). Conclusion: PPOS is an aggressive malignancy with a generally poor prognosis. Age, tumor size, and surgical resection were independent prognostic factors. Combined surgery and chemotherapy may offer additional survival benefit for selected patients. Larger multi-institutional studies are needed to validate these findings.
