Fixed-treatment duration venetoclax following covalent BTKi progression in chronic lymphocytic leukemia

Year: 2026
Type: article
Abstract: Covalent Bruton tyrosine kinase inhibitors (cBTKis) are commonly used targeted agents in early lines of therapy for chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL) 1,2 ; however, treatment following disease progression on cBTKis remains a challenge in clinical practice.3,4 Venetoclax is a B-cell lymphoma-2 inhibitor approved for CLL and SLL treatment, either as continuous monotherapy or as a fixed-treatment duration (FTD) regimen in combination with anti-CD20 antibodies (FTD Ven+anti-CD20; frontline, 12 months; relapsed/refractory, 24 months). 4,5enetoclax has demonstrated durable responses and outcomes among patients with previous cBTKi exposure, both in clinical trials (eg, M14 032 and VENICE-1) 6,7 and in real-world studies.[8][9][10][11][12][13][14][15] Although these prior analyses included a proportion of patients who discontinued their cBTKi due to disease progression, real-world evidence on venetoclax used specifically among patients who previously progressed on a cBTKi is limited.Although an analysis from the CLL Collaborative Study of Real-World Evidence (CORE) was conducted to assess clinical outcomes among patients who initiated venetoclax-based therapy after having stopped cBTKi for a variety of reasons (eg, intolerance and disease progression), it did not explicitly focus on patients treated with FTD Ven+anti-CD20 in post-cBTKi progression settings nor did it assess overall survival (OS).13 With the expanding CLL and SLL treatment landscape, most recently with the availability in the post-cBTKi setting of the noncovalent BTKi pirtobrutinib, 4 there is a need for real-world evidence on existing therapies such as FTD Ven+anti-CD20 after previous cBTKi progression. Therefore, this study was conducted to assess
