Parkinson’s disease (PD) is a common neurodegenerative disorder of the central nervous system in middle-aged and older adults. Its core pathological features include the degeneration and loss of dopaminergic neurons in the substantia nigra pars compacta and the abnormal aggregation of α-synuclein. Current therapies are unable to halt disease progression and often lead to adverse effects. Acupuncture, as a safe adjunctive treatment, shows significant potential in improving both motor and non-motor symptoms of PD. This review systematically summarizes recent literature and discusses the potential molecular mechanisms of acupuncture in treating PD from six aspects: α-synuclein aggregation and its clearance pathways, mitochondrial dysfunction and oxidative stress, neuroinflammation, anti-apoptosis, basal ganglia neurotransmitter regulation, and the brain-gut axis. We also summarize clinical evidence from randomized controlled trials and meta-analyses, construct an interactive network of related pathways, and analyze the limitations of current studies. The aim is to provide a reference for further mechanistic studies and clinical applications of acupuncture in the management of PD.
