Abstract: Non-invasive imaging of inflammation is critical for disease diagnosis, monitoring, and therapeutic evaluation. Nanobodies, owing to their small size, high stability, and ability to access otherwise inaccessible epitopes, are emerging as powerful imaging agents. However, conventional non-specific conjugation strategies often yield heterogeneous products that can limit reproducibility and in vivo performance. In the associated manuscript, “Farnesyltransferase Enables Modular Assembly of Dual-Functional Nanobody Conjugates for In Vivo Imaging of Inflammation,” a chemoenzymatic strategy is described for the precise and modular assembly of nanobody conjugates using farnesyltransferase (FTase)-mediated site-specific modification. A trifunctional FTase substrate was developed to introduce two orthogonal bioorthogonal handles, an aldehyde and an azide, onto nanobodies in a single enzymatic step. These handles enabled independent and controlled installation of imaging payloads, including fluorophores and radioisotope chelators, as well as pharmacokinetic modifiers such as PEG. Applying this strategy to nanobodies targeting CD45 and CD11b, markers broadly expressed on immune cells and myeloid subsets, respectively, generated well-defined, dually modified constructs for high-contrast visualization of immune organs and inflamed tissue in vivo.
