Abstract: Anxiety disorders constitute the most prevalent category of psychiatric disorders, and numerous clinical practice guidelines have been developed to inform their treatment. The objective of this systematic review was to synthesise and compare clinical practice guideline recommendations for the treatment of social anxiety disorder, panic disorder, and generalised anxiety disorder. I searched MEDLINE and Guidelines International Network to identify guidelines. Clinical practice guidelines that addressed the treatment of any of the three disorders, were published between 2011 and 2025, and were available in English, Finnish, or Swedish were eligible. I excluded guidelines that were limited to specific treatments, settings, or populations. I assessed guideline quality and risk of bias qualitatively. A total of 17 guidelines from 12 countries met inclusion criteria. Guidelines recommended a wide range of psycho- and pharmacotherapies. Recommendations were largely consistent for first-line treatments, with cognitive behavioural therapy and selective serotonin reuptake inhibitors recommended first-line for each of the three disorders. Substantial variability was observed in second- and third-line recommendations. Variability in psychotherapy recommendations may reflect regional differences in practice and service availability, while inconsistencies in pharmacotherapy recommendations may be attributed to differences in the way in which guidelines considered adverse effect profiles and weak evidence. Transparency and methodological quality varied widely between guidelines and likely further contributed to inconsistencies in treatment recommendations. The findings emphasise the continued importance of clinical judgement when using clinical practice guideline recommendations to inform the treatment of anxiety disorders.
