Abstract: Despite successful percutaneous coronary intervention (PCI), recurrent angina remains a significant clinical problem. This prospective randomized controlled study included 120 patients who had undergone successful PCI. The patients were randomized into two groups of 60 people each. The main group received standard therapy in combination with colchicine at a dose of 0.5 mg/day, while the control group received only standard therapy. The follow-up period was 12 months. The primary endpoint was the frequency of recurrent angina. Secondary endpoints included achievement of target LDL-C levels, changes in inflammatory markers (hs-CRP, IL-6), quality of life (SF-36 and HADS questionnaires), and the frequency of major adverse cardiovascular events (MACE). After 12 months, recurrent angina occurred in 7 (11.7%) patients in the main group compared with 17 (28.3%) patients in the control group (p = 0.006; OR = 0.41; 95% CI 0.19–0.89). The main group showed a statistically more pronounced decrease in hs-CRP (−38.4% vs −12.7%, p < 0.001), a higher rate of achieving the target LDL-C level < 1.4 mmol/L (68.3% vs 41.7%, p < 0.01), and greater improvement in the physical component of health according to the SF-36 scale (+18.4 vs +9.7 points, p < 0.01). The MACE rate was lower in the main group (8.3% vs 18.3%, p = 0.047). Colchicine was well tolerated. The addition of low-dose colchicine to standard therapy significantly reduces recurrent angina, suppresses residual inflammation, and improves long-term prognosis in patients after PCI.
