Abstract: Insulin deficiency, as occurs in type 1 diabetes and advanced stages of type 2 diabetes, remains a major medical challenge because current insulin-based therapies do not fully restore metabolic homeostasis and can cause severe hypoglycemia. This paper shows that insulin deficiency profoundly remodels hepatic mRNA translation, globally suppressing protein synthesis while selectively activating metabolic pathways. Using ribosome profiling and functional studies, we identify regenerating islet-derived protein 3α (REG3A) as a leptin-responsive hepatic factor that improves hyperglycemia independently of insulin signaling. These findings identify translational control as an adaptive layer of metabolic regulation in insulin deficiency and reveal a potential insulin-independent pathway for improving glucose homeostasis.
