Abstract: Drug-resistant epilepsy is one of the most complex problems in modern clinical neurology, epileptology, and functional neurosurgery. Structural forms of the disease are of particular importance, as epileptic seizures in these cases are associated with a morphologically identifiable brain lesion. A retrospective clinical and neuroimaging study was conducted and included 120 patients with structural drug-resistant epilepsy. All patients underwent clinical and anamnestic assessment, neurological examination, electroencephalography, and brain magnetic resonance imaging. The most common structural causes were mesial temporal sclerosis, focal cortical dysplasia, benign epilepsy-associated tumors, post-traumatic gliosis, cavernous malformations, and post-stroke lesions. Mesial temporal sclerosis was found to be more frequently associated with focal impaired-awareness seizures and temporal auras, focal cortical dysplasia with early disease onset and seizure polymorphism, and post-traumatic and post-stroke lesions with more pronounced focal neurological deficits. The findings emphasize the need for comprehensive assessment of the structural substrate, clinical semiology, and functional status of the patient when planning diagnostic and therapeutic strategies.
