Abstract: Incretin-based therapies are increasingly used for obesity management, raising debate about whether they could challenge or replace metabolic bariatric surgery (MBS). Here, we compare evidence for both treatments, focusing on effectiveness, limitations and costs. We also explore how obesity care pathways may evolve and highlight key evidence gaps that must be addressed to define the future roles of incretin-based therapies and MBS in obesity management.
