Abstract: Background This study analyzes the vertical relationship of maxillary posterior teeth and sinus floor, evaluating age- and gender-related variations and their role in odontogenic sinusitis using CBCT.Methodology A retrospective analysis of 225 CBCT scans (115 females, 110 males) was conducted. Distances between root apices of premolars and molars and the sinus floor were measured bilaterally. Data were classified using Didilescu et al.‘s system and analyzed across age and gender groups using ANOVA, chi-square, and independent t-tests.Results A significant association was observed between age and the vertical relationship of maxillary posterior teeth to the maxillary sinus floor, with decreasing root–sinus distance in older individuals. Palatal roots of maxillary molars showed the closest proximity to the sinus floor, and Type 1 relationship (0–2 mm) was the most common pattern. No significant sex-based differences were identified. Conclusion The vertical relationship between maxillary posterior teeth and the maxillary sinus floor is significant in odontogenic sinusitis. The reduced root-sinus distance with advancing age enhances sinus involvement, particularly in the molar areas. CBCT is an essential instrument for assessing these interactions and enhancing therapy planning.Practical Implications CBCT-based assessment of dento-sinus proximity aids accurate diagnosis, guides safer extractions and implant planning, minimizes sinus perforation risk, and supports prevention of odontogenic sinusitis, especially in older patients with reduced distances.
