Acromioclavicular Joint Injections: Comparing Joint Identification Using Palpation Versus Ultrasound

Year: 2025
Type: article
Study design: Observational Study
Abstract: Objective The purpose of this study was to compare palpation-guided versus ultrasound-guided identification of the acromiclavicular (AC) joint. The study also evaluated whether other factors, including age, Body Mass Index (BMI) and gender, influenced the accuracy of palpation identification. Design This was a prospective-controlled study. Once shoulder measurements were obtained, no further study design procedures were required. Based on prior research, the study assumed that ultrasound was nearly 100% accurate in identifying the AC joint. Setting: This study was performed in an academic University Emergency Department and Primary Care Sports Medicine Clinic. Participants Participants were selected using convenience sampling from the above settings. 18 participants total (9 male, 9 female) were recruited. Those less than 18 years old, those lacking decision-making capacity, those with evidence of trauma to the shoulder or those who had undergone shoulder surgery within the previous twelve months were excluded. Interventions None. Main Outcome Measures Distance between the palpated AC joint and the nearest border of the AC joint as measured by ultrasound, in millimeters lateral or medial. Results The raw mean distance from the palpated AC joint to the nearest margin on ultrasound was 8.62mm. The odds ratio of successfully entering the joint through that mark was 3.00. Moreover, neither age, BMI nor gender had any association with the distance. Conclusion Though relatively quick, the palpation-guided approach to AC joint identification is inaccurate. Compared to ultrasound guidance, a practitioner is more likely to miss the joint space and cause additional patient discomfort. Where possible, providers should perform AC joint injections under ultrasound guidance.
