Abstract: The paper presents a retrospective analysis of the treatment of 221 patients with acute biliary pancreatitis. When a stone is impacted in the major duodenal papilla, endoscopic papillotomy in the first hours is recommended; in choledocholithiasis, obstructive jaundice, cholangitis and acute biliary pancreatitis - endoscopic papillotomy and lithoextraction performed within the first 24 hours after the patient's admission to the clinic. Cholecystectomy, as a rule laparoscopic, is expedient to perform after conservative resolution of mild biliary pancreatitis within the following 3-7 days. After endoscopic papillotomy has been performed, carrying out cholecystectomy without discharge from the hospital is also justified if the procedure was completed without complications. In acute biliary pancreatitis complicated by sterile or infected fluid collections, cholecystectomy should be postponed until their complete resolution and the elimination of the systemic inflammatory response.
