Simulation-based Training for Respiratory Teams in an Intermediate Respiratory Care Unit: From Evidence to Local Implementation

Year: 2026
Type: article
Abstract: Intermediate respiratory care units (IRCU) require rapid clinical judgement, advanced respiratory skills and effective multiprofessional teamwork. Because traditional workplace-based training is variable and exposure to critical events is unpredictable, simulation can provide a safe, structured environment for practising technical and non-technical competencies. We developed a simulation-based training programme for IRCU professionals from three hospitals, including nurses, pulmonology residents and consultant pulmonologists. The programme followed the SimZones framework, progressing from online preparatory material and hands-on workshops to increasingly complex team-based scenarios in a simulation centre. Activities addressed non-invasive ventilation, patient-ventilator asynchronies, mechanical insufflation/exsufflation, tracheostomy care, bronchoscopy during non-invasive ventilation, communication errors and exploration of patients' values and preferences regarding life-sustaining treatment. Baseline perceptions of interprofessional learning were assessed using SPICE-R2. This type of initiative has the potential to support patient safety culture.
