Abstract: Pregnant women and newborns are vulnerable to sexually transmitted infections (STIs), even in the absence of high-risk sexual behavior. One STI associated with adverse pregnancy outcomes is syphilis, caused by the bacterium Treponema pallidum. Maternal-to-fetal transmission of syphilis is often associated with poor outcomes when maternal infection remains undetected or is inadequately treated during early pregnancy. This case report aims to evaluate intrauterine fetal death (IUFD) in a pregnant woman with latent-stage syphilis.A 20-year-old primigravida at 26 weeks and 6 days of gestation was diagnosed with G1P0000, 26 weeks and 6 days of gestation, intrauterine fetal death (IUFD), and latent-stage syphilis. The diagnosis of syphilis was established based on a positive Venereal Disease Research Laboratory (VDRL) test (titer 1:2) in the absence of mucocutaneous manifestations. The patient received a single intramuscular dose of 2.4 million IU benzathine penicillin G. The diagnosis of IUFD was confirmed upon admission to the emergency obstetric unit at Wangaya Regional General Hospital, and the delivered fetus was found to have multiple congenital anomalies. Undiagnosed and inadequately treated maternal syphilis can result in perinatal death due to multiple congenital anomalies.
