Abstract: We herein report a rare case of non-IgM lymphoplasmacytic lymphoma (LPL) and acute myeloid leukemia (AML). A 64-year-old man with MYD88 L252P-positive LPL developed inv(16)-positive AML during LPL follow-up. Chemotherapy eradicated both AML and LPL cells from the bone marrow. Whole-exome sequencing revealed distinct mutation profiles with no shared mutations between the LPL and AML clones. These data suggest the coincidental development of genetically independent clones rather than a common ancestral origin. Our findings indicate the need for genomic stratification in dual hematological malignancies to accurately distinguish between coincidental and clonally related diseases.
