Pneumocystis and its interaction with the lung microbiota and microbiome: an integrative narrative review

Year: 2026
Type: article
Pneumocystis and its interaction with the lung microbiota and microbiome: an integrative narrative review Abstract: The lung, traditionally considered a sterile organ, is now recognized as a complex microbial ecosystem harboring a diverse microbiota with structural and immunomodulatory functions. In this context, Pneumocystis jirovecii, an atypical opportunistic fungus historically associated with infections in immunocompromised patients, has emerged as a potential modulator of the lung microbiome, even in immunocompetent individuals. This integrative narrative review analyzes the available evidence on the interaction between Pneumocystis, the microbiota, and the lung microbiome. A systematic search of international biomedical databases was conducted up to September 2025, selecting relevant original studies. The findings indicate that Pneumocystis infection or colonization is associated with a reduction in lung microbial diversity and a reorganization of the microbial community. At the functional level, alterations in important metabolic pathways are observed. Taken together, the results suggest that P. jirovecii could act as an ecological and immune-metabolic modulator of the lung microbiome. We propose moving toward integrated multi-omic studies to establish causal relationships and evaluate the diagnostic and therapeutic potential of this interaction in chronic respiratory diseases and opportunistic infections.
