Abstract: Background: Circulation of CSF is important for normal intracranial physiology. Changes in CSF dynamics have been attributed to many neurological diseases including generalized cerebral atrophy. Non-invasive quantitative assessment of CSF flow is available by phase-contrast magnetic resonance imaging (PC-MRI). In the present study, CSF flowmetry parameters were assessed in normal subjects and patients with generalized cerebral atrophy, and the clinical significance was evaluated. Materials and Methods: In a prospective comparative study, we included 40 patients with generalized cerebral atrophy (diagnosed by MRI) and 40 healthy controls. For CSF flow measurement, all subjects had quantitative phase-contrast MRI at the cerebral aqueduct. Peak systolic velocity, peak diastolic velocity, average flow rate, stroke volume and net flow volume were measured and compared among groups. Results: Patients with generalized cerebral atrophy had significantly larger aqueduct stroke volume (48.6 ± 12.4 µL/cycle) than controls (31.8 ± 8.7 µL/cycle, p<0.001). Peak systolic velocity and net CSF flow were also significantly increased in the atrophy group. There was a positive correlation between the stroke volume of the aqueducts and the enlargement of the ventricles. The Phase-contrast MRI technique was effective in quantifying changes in CSF dynamics related to cerebral atrophy. Conclusion: An increase in CSF flow dynamics is well correlated with generalized cerebral atrophy. Ventricular enlargement and CSF circulation changes in cerebral atrophy could be useful imaging biomarkers in terms of MRI flowmetry parameters derived from the phase-contrast MRI technique.
