Management of Concomitant Complicated Crown and Crown-Root Fractures in an Adolescent: A Case Report

Study design: Case Report
Abstract: This case report describes the multidisciplinary management of concurrent complicated crown and crown-root fractures in a 13-year-old male, categorized under the updated WHO ICD-11 framework. The patient presented seven days post-trauma with a complicated crown fracture on tooth 11 (ICD-11: NA0D.03) and a complicated crown-root fracture on tooth 21 (ICD-11: NA0D.05) featuring a subgingival mobile fragment. Because radiographs showed early periapical radiolucency, immediate endodontic therapy was initiated. The mobile palatal fragment of tooth 21 was temporarily reattached with composite to accommodate an urgent social function. Following a 15-day interim period that confirmed periapical healing, permanent endodontic obturation was completed. Definitive restorations included a direct composite buildup for tooth 11, and a fiber post with a core buildup for tooth 21, both finalized with zirconia crowns. This well-sequenced approach of provisional fragment reattachment, endodontic disinfection, and definitive biomimetic restoration predictably preserved the traumatized teeth and maintained alveolar bone integrity in a growing adolescent.
