Abstract: It has been suggested that MRI has a relevant role in guiding the surgical extent of breast cancer by measuring the size of residual tumor after neoadjuvant chemotherapy; likewise, it has also been shown that MRI has a good sensitivity for residual disease detection. Our primary objective was to evaluate the accuracy of MRI in the assessment of residual disease after neoadjuvant chemotherapy in patients with breast cancer. Seventy seven women with breast cancer treated at Grupo Oroño Mastology Unit were evaluated; all the patients underwent MRI before and after neoadjuvant chemotherapy. The accuracy of breast MRI for detecting residual disease was 95%. The response correlation between breast MRI and pathology was very high. MRI partial response rates for HER2- and triple-negative luminal tumors ranged from 60 to 70%; lower rates for HER2- and triple-negative tumors ranged from 22% to 38%.The presence of residual disease in breast cancer patients treated with neoadjuvant chemotherapy could be accurately assessed using breast MRI.
