Bridging Treatment and School: A Clinic-Based Transition Model for Young People With Emotionally Based School Avoidance

Year: 2026
Type: preprint
Abstract: Some young people stop attending school not because they will not go, but because they cannot. For those whose non-attendance is driven by intense anxiety, low mood or overwhelming distress at the point of entry, weekly outpatient therapy is often not enough, while hospital admission is too much. This practice paper describes a clinic-based bridging programme in Singapore: a time-limited, one-to-one therapeutic day transition programme that combines daily academic work, counselling, social skills and executive functioning coaching, parent coaching and active school coordination. Its distinguishing feature is a set of explicit decision rules for progression. A step is increased only once it is stable, reduced when it exceeds the young person’s tolerance, and reviewed weekly; the first step is matched to the element the young person most fears, and the same graded logic is applied first within the centre and then within the school. The paper specifies the model using TIDieR headings, presents a logic model, traces its development through four founding cases, and reviews the evidence behind each component. It argues that the model offers a specifiable middle-tier option that includes young people with neurodevelopmental presentations, a group often excluded from intensive programmes, and outlines a research agenda. No outcome data are yet available, and the model’s effectiveness remains to be tested.
