Review of antibiotic prescribing in fluoroscopic procedures and indwelling tubes in paediatric urology

Abstract: There is very little evidence-based practice when it comes to antibiotic usage in fluoroscopic procedures, instrumentation and indwelling catheters, particularly in children. Most guidelines still suggest antibiotics for these procedures but the need for antibiotic has been increasingly debated. Antibiotic stewardship is vital and awareness of the danger of antimicrobial resistance is essential. This paper focuses on evidence of antibiotic usage and utility in procedures involving fluoroscopy and urinary tract instrumentation and usage related to indwelling catheters and stents. A comprehensive literature search was conducted across six electronic databases and one clinical trial register. Forty-four studies met the inclusion criteria and were included in the systematic review. There is increasing evidence that antibiotics should be limited to selected groups of children. Judicious prescribing in high-risk groups may be the way forward, but stronger evidence is necessary to identify those groups and make recommendations.
