OsteoStrong and dynamic exercise in older women: rethinking comparative effectiveness

Year: 2026
Type: article
Abstract: Exercise is a cornerstone of osteoporosis management, yet the comparative effectiveness of isometric machine‑based training versus dynamic multicomponent exercise remains unclear. Lindberg and colleagues conducted a randomized trial comparing OsteoStrong (OS) to dynamic multicomponent exercise (DME) in 194 older women with osteopenia or osteoporosis, finding superior improvements in gait speed, back endurance, and balance with DME. While these findings are clinically relevant, several methodological considerations warrant attention: the six‑fold difference in training volume (20 vs. 120 min/week) confounds modality comparisons; functional outcomes were interpreted as superiority evidence despite the trial's non‑inferiority design; participants' high baseline activity may limit generalizability; and unblinded follow‑up assessments risk detection bias. We offer a perspective on these methodological challenges and proposes directions for future research to refine exercise recommendations for this population.
