Anterior Segment Ischemia and Neurotrophic Keratopathy Following Ultrasound Cycloplasty in Acute Angle-Closure Glaucoma

Abstract: Ultrasound cycloplasty (UCP) is a minimally invasive, non-incisional glaucoma procedure that has emerged in recent years and is generally associated with few complications. We report a case of a patient with acute angle-closure glaucoma underwent UCP for IOP control. She developed iris neovascularization and neurotrophic keratopathy(NK)at postoperative week 6, followed by hypopyon, flattening of the anterior chamber, and thinning of the peripheral cornea at postoperative week 11, 12, and 14, respectively. We concluded that UCP can lead to anterior segment ischemia (ASI) and NK, which manifest as iris neovascularization, painless corneal epithelial defects, peripheral corneal thinning, hypopyon, and flattening of the anterior chamber. Preoperative screening for high-risk factors is essential before performing UCP.
