Abstract: Background: Children's equino cavo varus deformity is a complicated foot deformity brought on by an imbalance in the muscles and hyperactivity of invertor muscles like the tibialis anterior and posterior. It can cause gradual functional limitation, difficulty wearing shoes, and disruption of gait. In situations that are resistive or inflexible, surgical treatment is frequently necessary to restore a plantigrade, painless, and functional foot. Methods: This prospective randomized study was conducted on 12 children with equino cavo varus foot deformity at Zagazig University . Patients aged 4–11 years underwent combined lateral column shortening and tibialis anterior tendon transfer. Clinical, radiological, and functional assessments were performed preoperatively and postoperatively, with follow-up for one year. Statistical analysis was performed using SPSS version 27. Results: The mean age was 8.25 ± 1.96 years. Significant improvement was observed in radiological parameters including lateral talo-first metatarsal angle, anteroposterior talo-first metatarsal angle, and calcaneal pitch angle (p = 0.001). Postoperatively, 91.7% of patients showed improved walking ability. According to Palmar & Garceau criteria, 83.3% achieved excellent or good outcomes. Kling score showed satisfactory results (excellent or good) in 83.3% of patients. Recurrence occurred in 16.7% and overcorrection in 8.3% of cases. Conclusion: Combined lateral column shortening and tibialis anterior tendon transfer is an effective surgical technique for correction of equino cavo varus deformity in children, providing significant clinical and radiological improvement with acceptable complication rates.
